Waiver of Liability/Release

By checking the box, I acknowledge and agree that I have read the following and know of the nature of the activities in Jennifer Moore’s yoga classes and I agree to all the terms of this Waiver of Liability/Release as stated below.

I certify that each person practicing yoga in my household with Jennifer Moore and Breathe Moore LIVE Yoga has created their own individual account with Breathe Moore LIVE Yoga and therefore has agreed to all of the terms of this Waiver of Liability/Release.

I am aware that Jennifer Moore and Breathe Moore LIVE Yoga are here to serve me by sharing Jennifer Moore’s knowledge of yoga and health which is not all encompassing.

I recognize that yoga requires physical activity, which may at times be strenuous.

By my participation in the LIVE Zoom classes or REPLAYS with Jennifer Moore and Breathe Moore LIVE Yoga, I certify that I am physically fit and agree to take full responsibility for not exceeding my limits in the practice of yoga.

Jennifer Moore and Breathe Moore LIVE Yoga strongly urge me to consult my physician before participating in any physical activity. It is my doctor’s and my responsibility to ascertain that there is no medical reason to prevent my participation in Jennifer Moore’s classes.

If I am pregnant, I will make this fact known to Jennifer Moore and Breathe Moore LIVE Yoga (prior to taking class) via written email so that Jennifer Moore may consult with me on yoga modifications for pregnancy. I acknowledge that Jennifer Moore’s knowledge of pregnancy yoga modifications is not all encompassing.

If I am injured prior to taking Jennifer Moore’s classes, I will make this fact known to Jennifer Moore and Breathe Moore LIVE Yoga via written email so that Jennifer Moore may consult with me on yoga modifications for my injury. I acknowledge that Jennifer Moore’s knowledge of yoga injury modifications is not all encompassing.

I expressly acknowledge that I assume full risk for any injuries that I may incur, whether caused by me or Jennifer Moore or Breathe Moore LIVE Yoga.

I expressly acknowledge that I, my heirs, successors, and assignees indemnify and hold harmless Jennifer Moore and Breathe Moore LIVE Yoga from any liability and any and all claims arising out of personal injury, including, without limitation, personal, bodily, or mental injury, death, economic loss, property loss or any damage to me, my spouse, guests, unborn children, or relatives resulting from any negligence of Jennifer Moore and Breathe Moore LIVE Yoga.

I agree to indemnify, defend and hold Jennifer Moore and Breathe Moore LIVE Yoga harmless against any liability, damages, defense costs, including attorneys’ fees, or from any other costs incurred in connection with claims for bodily injury, wrongful death lack of society/consortium, property loss or property damage caused by my own negligence or other wrongful acts or omissions.

I understand and acknowledge that Jennifer Moore and Breathe Moore LIVE Yoga are providing recreational services and may not be held liable.

If I am under 18 years of age, my parent or guardian has read these terms and will click the box for me thus agreeing to all terms on my behalf.

By checking the box, I acknowledge and agree that I have read the foregoing and know of the nature of the activities in Jennifer Moore’s yoga classes and I agree to all the terms of this agreement and Jennifer Moore’s and Breathe Moore LIVE Yoga’s policies.
